Copy of Mortgage Statement or A fully Executed Lease Agreement/Letter from Landlord
Copy of a major Utility Bill (example Gas; Electric;Telephone)
Notarized Affidavit of Residency
Medical Records
Language Survey
Student's Discipline Record from prior school
